2023-2024 AWANA Registration - PreSchool through 5th Grade

This form is for children ages, 3 years old (or 2 years prior to Kindergarten) through 5th Grade. This program runs weekly, on Wednesday nights, September until May. AWANA is an international children’s ministry that teaches children that God is real, He loves them, and He has expressed that love through His Son, Jesus Christ. The goal of AWANA is to give boys and girls the chance to know, love and serve God. Our AWANA program, here at First Baptist in Colville, runs weekly, September through May and currently offers AWANA’s different programs for different age groups. All groups enjoy Bible lessons, verse memory, songs, games, and, of course, snack time.

I do hereby authorize the treatment by a qualified and licensed medical doctor of the minor listed in the form herein in the event of a medical emergency which, in the opinion of the attending physician, may endanger his or her life, cause disfigurement, physical imparirment or undue discomfort if delayed.
This authority is granted only after a valid effort has been made to reach me.
This release is completed by my own free will with the sole purpose of authorizing emergency medical treatment for the child listed herein.

Every activity sponsored by this church is carefully planned and adequately supervised by mature adults. However, even with the best of planning and precaution, unforseen events can occur. By completing this form, the parent or guardian agrees to assume and accept all risks and hazards inherent in church-related social activites. They also agree not to hold this church or its employees or volunteer assistants liable for damages, losses or injuries to the person or property undersigned. The parents or guardians understand that they are signing for the minor listed on this form and the signature is for both a medical and liability release.
